Emotional Support Dog Registration for Watson

Watson is an insulin dependent diabetic.  He uses Caninsulin twice a day 6Am and 5:30Pm Watson has pancreatitis so is on a vegetarian dry kibble diet. Watson is up to date with all of his vaccines. DOB: April 6th, 2011.  Watson is microchipped and tattooed. Watson is a neutered male. Watson is often wearing a libre freestyle glucose monitor that is just below the skin, but is only active for 14 days at a time.  Watson will need his glucose checked before administration of insulin. He is on a sliding scale 15 and above 7.5 units 14.9 and below to 10.0  7 units of insulin and 9 as a blood glucose reading  to 5 reduce by 2units. 5 and below no insulin at all
